UnitedHealth earnings: date, guidance and results
UnitedHealth's next results are expected on January 20, 2027. Watch whether medical costs improve without modernization simply moving expense elsewhere.
What has UnitedHealth guided for the year?
Management says the higher earnings floor already includes investment late in 2026. Its longer-term growth target is therefore an ambition to turn the recovery into a repeatable business, not proof that one strong quarter will continue. Analysts must decide whether the remaining members are better priced and whether medical costs stay controlled.
| Period it covers | Low | High | Date it was given | |
|---|---|---|---|---|
| Adjusted diluted earnings per share | Year to Dec 2026 | 20.50 | Oct 13, 2026 | |
| UnitedHealthcare, Adjusted Operating Earnings | Year to Dec 2026 | 12,000 | Jul 16, 2026 | |
| Optum Health, Adjusted Operating Earnings | Year to Dec 2026 | 2,215 | Jul 16, 2026 | |
| Medical Care Ratio | Year to Dec 2026 | 87.9% | 88.4% | Jul 16, 2026 |
| long-term target growth range | 2027 and beyond | 13.0% | 16.0% | Oct 13, 2026 |
What this means. The 8,000 million shown for dividends paid means the company expects to pay more than 8 billion dollars, not exactly 8 billion dollars.
Did UnitedHealth keep its past guidance?
In July, Hemsley restored guidance of at least US$16.00, and the year ended at US$16.35. On October 13, 2026, DeVeydt raised the outlook to at least US$20.50. That is evidence of improvement, but it does not erase the original failure to recognize rising costs quickly enough.
How have UnitedHealth revenue and profit trended?
September 2026 net income rose from $2.3 billion a year earlier to $4.1 billion, while revenue slipped from about $113.2 billion to $112.7 billion. Earning more on slightly less revenue is better evidence than merely processing more premium dollars.
The sequence also matters. December 2025 was depressed by concentrated year-end charges, while June and September 2026 improved. Across nine months, removing identified portfolio and restructuring effects still leaves operating earnings growth. The recovery therefore survives those adjustments, but it must now survive medical-cost pressure and membership reductions.
| Sep 2026 | Jun 2026 | Mar 2026 | Dec 2025 | Sep 2025 | Jun 2025 | Mar 2025 | Dec 2024 | |
|---|---|---|---|---|---|---|---|---|
| Core earnings (company's measure) | 4,228 | 5,792 | 6,578 | 1,924 | 2,650 | 3,716 | 6,607 | 6,311 |
| Net income | 4,110 | 5,484 | 6,280 | 10 | 2,348 | 3,406 | 6,292 | 5,543 |
| EPS, diluted | 4.56 | 6.04 | 6.90 | 0.01 | 2.59 | 3.74 | 6.85 | 5.98 |
| Adjusted diluted earnings per share | 4.69 | 6.38 | 7.23 | 2.11 | 2.92 | 4.08 | 7.20 | 6.81 |
What this means. The company's core earnings are higher than reported net income because its own measure leaves out unusual costs and losses included in the reported profit.
What should I watch in UnitedHealth's next results?
The fuller outlook should show whether management’s growth signal survives a detailed budget. Investors should read the medical care ratio alongside the operating cost ratio: treatment costs must improve without modernization simply moving expense elsewhere.
