What has Cigna guided for 2026?

The company reaffirmed a target of at least $30.45 in adjusted earnings per share for 2026. Earnings per share divides profit among shareholders; adjusted earnings remove items management considers less representative of normal operations. In the second quarter, ordinary profit was $6.29 per share, while adjusted profit was $7.78 because Cigna excluded items such as acquired-intangible amortization, investment results and special charges. That can clarify recurring performance, but restructuring still costs real money.

Cigna Healthcare, the insurance business, is expected to provide at least $4,550 million of pre-tax adjusted operating income. Evernorth, which houses pharmacy and care services, is expected to provide at least $6,900 million. Yet pharmacy revenue rose 8% in the second quarter while Pharmacy Benefit Services’ adjusted operating income fell 27%. More prescriptions or more expensive drugs can increase revenue without increasing the fee left as profit.

What this means. The $500 million in annualized after-tax savings is a yearly cost reduction, not a one-time payment, and some of it was already achieved in 2025.

Did Cigna keep its earnings promise?

Cigna did meet its shorter promise. Management guided to at least $29.60 in full-year adjusted earnings per share and delivered $29.84. “Adjusted” means Cigna excludes specified items from accounting profit, so meeting that target does not erase a miss on a longer-term measure.

How have Cigna's revenue and profit trended?

Core earnings increased about 6.4% from the prior year, while remaining almost unchanged from March. The quarterly series therefore suggests moderate improvement, not a steadily accelerating business.

Evernorth’s divisions moved in opposite directions. Pharmacy Benefit Services generated $34,496 million of revenue and $609 million of adjusted income. Specialty and Care Services produced $1,054 million of income. Drug mix lifted pharmacy revenue even as pharmacy activity declined, while specialty results benefited from Shields and operating efficiencies.

From the full memo · 1. The business

Cigna is a company that sells ways to pay for, organize and deliver health care. A dollar enters through premiums, employer and health-plan fees, or payments for medicines and care, then leaves for claims, drugs, providers and operating costs; what remains is the business’s profit.
